Problem

PROBLEM The project aims at Home Care Nursing Services, in such a way to follow up and to bridge the gap between all discharge patients both from hospitals, health posts, clinics and out-patients, and Nursing Care in their various Homes in south eastern part of Nigeria , with effective, efficient and knowledge based Nursing home care services. To reduce relapses of discharged patients to its barest minimum within the period of 6month of home care nursing in south eastern part of Nigeria,and reducing the cost o re-admission. To Promoting Preventive Medicine, Increase the Awareness of Home care Nursing Services in all communities and homes in South eastern part of Nigeria . To reduce the length of patients stay in various level of admissions through promoting early discharge and hence reducing cost of admission, and finally to encourage patients self health dependency. To promote and integrate TeleNursing in care aspect of home care nursing in south eastern part of the country.

This is very encouraging knowing that our patients need nursing care even after discharge from hospital and nurses are not disposed to this duty outside the hospital contact with these patients. All nurses should support this move.

Great idea!! Home care in the US is regulated. What are the rules that regulate this practice in Nigeria? We need rules to prevent abuse of the patients and the practice. Regulations hold people to standards of practice. It will also assist in the development of training programs for Home Care Nurses.
